For your convenience, we have posted several forms you
will need throughout the year, including the Guest Player 
Release Form, and the NTSSA Full Release Form.

These forms are presented in two formats - Microsoft Word and Adobe Acrobat

bulletMICROSOFT WORD FORMS:  Right click; click on "Save Target As"; save it to your computer (remember where you saved it); open it in MS Word and type on the form.

Team Fact Sheet and Log Sheet – This form is mandatory for competitive teams. You are to provide North Texas with a copy of your team fact sheet no later than June 15. It is imperative that all information be accurate. You are to provide a copy for every parent and player that attends your team tryouts. Both player and parent must sign the log sheet acknowledging that they received a copy of the fact sheet. You must give them a copy to take with them, not just show them a copy.

Competitive USYS Membership Form (Word format)
This is the player "contract." This form cannot be signed by a player or parent before July 10.

NTSSA Roster  (Word format)
Roster needs to be completed. List all players’ birth names (not nick names). Email addresses are required. A parent’s email address is acceptable.

NTSSA Add and Delete Form (Word format)
This form is used when you need to make changes to your roster. Transfers must be approved by NTSSA. If you want to transfer a player to your team, call me before you do anything.

Background Check Form (Risk Management) (.pdf format only) – This form is to be filled out by every Coach, Assistant Coach, Manager, Trainer, etc. A copy of each person’s drivers license must be attached. The form will not be accepted if it is not completely filled out.

Medical Release (Word format)
Every player that plays competitive soccer must have a notarized medical release. Make copies and put the originals in a safe place. You should have the medical releases with you at all games and practices. Never give the original to anyone. Some tournaments ask to see the original medical releases. If a tournament requires that you turn one in, show them the original and turn in copies.

Guest Player Release (Word format)
Be sure you are familiar with North Texas Rule 4.7 concerning guest players.  http://www.ntxsoccer.org/Bylaws. Also remember that during the month of June, guest players still have to have a valid guest player release signed by their coach in order to guest play with any team.  

NTSSA Full Release Form (Word format)
 

REQUIRED REGISTRATION PAPERWORK - BRING TO REGISTER YOUR TEAM:

  1. Completed USYS Membership Form for each player (completely filled out)   
  2. Birth Certificate for each player (only a copy of the certified state-issued birth certificate will be accepted. Hospital-issued birth certificates will not be excepted, per North Texas Rules ). Passports and drivers licenses are acceptable for proof of age.  
  3. Roster completely filled out, signed and dated by coach on both sides.
  4. North Texas Risk Management Form (required for every coach / asst. coach / manager / trainer/ along with a copy of DL)
  5. Copy of Team Fact Sheet Log
  6. Check for $30.00 per player

Make copies of your Membership forms and Birth Certificates to keep for your records.

Registration will start the evening of July 11, 2005.  
You will need to call for an appointment (see numbers at top of this page)
